The Division of Human Genetics offers a two-year accredited fellowship in Medical Genetics. Applicants should have an MD degree and should have completed a three-year U.S. residency in Pediatrics, Internal Medicine, Family Medicine or OB-GYN. The Medical Genetics Fellowship is focused on preparing candidates for an academic career emphasizing clinical genetics, teaching and clinical research. The program accepts one new fellow every other year.
Training is accomplished by combining a variety of intensive, integrated clinical rotations, during which the fellows become increasingly responsible for patient care, medical genetics coursework, focused laboratory rotations (cytogenetics, molecular genetics, and biochemical genetics) and several months of research opportunities.
The fellows will accomplish many of their learning objectives during their clinical rotations, including those in the general genetics (5½ days/week), prenatal and prenatal genetics and a variety of specialty programs throughout the five year program. The patients seen in these programs include a wide range of diagnoses in adults and children, most of whom are seen as consultations, however approximately 10% of our patients are seen for treatment and management. Both new and follow-up patients are assigned to the fellows on a regular basis. The NICU/newborn consultation service includes several hospitals in the greater Hartford region, with approximately 2-3 new in-patient consultations per week.
During the Cytogenetics rotation, the fellows will become familiar with a full range of laboratory techniques, including peripheral blood and tissue preparation for cytogenetic analysis, tissue culture techniques, amniotic fluid cell preparation, CVS preparation, microscopic analysis, FISH technology and automated karyotyping. They will also learn about different staining techniques, and the indications for their use. They will learn cytogenetics nomenclature and become familiar with cytogenetic interpretation and its application to clinical management. A detailed Cytogenetics Course is provided to the fellows bi-annually by Dr. Peter Benn, Laboratory Director.
The Molecular Genetics curriculum provides increasing levels of responsibility for the fellow, beginning with the didactic graduate course, clinical applications and then the rotation in the Genzyme Genetics laboratory. These include understanding of laboratory techniques with respect to molecular diagnosis, including PCR, ASO, comparative genomics, and methylation studies, as well as analysis of family pedigrees. Interpretations of laboratory test results are discussed at the laboratory rounds; implications of laboratory findings are discussed as part of this intensive rotation.
The Biochemical Genetics curriculum is focused on a 3-4 day rotation at the Yale University Medical School biochemical genetics laboratory, as well as the continuous experience of managing patients with a wide variety of biochemical and metabolic disorders followed by the UCHC Division of Human Genetics program.
It is essential that the fellow acquires skills and knowledge of the impact of genetic disorders across all age groups. Genetic diagnosis has the potential of involving an entire family. The fellowship achieves this objective in several ways.
The General Genetics Service accepts patients of all ages for genetic consultation. These patients and their families provide a wide variety of genetic diagnoses for the resident to build an extensive knowledge base. The Prenatal Genetics Service permits the fellow to diagnose and manage disorders before, during and after pregnancy under the supervision of board certified MFM physicians and genetic counselors, as well as a clinical geneticist.
The Hereditary Cancer Program is a multidisciplinary program with specialists from medicine, hematology-oncology, radiology, nursing and pathology and is coordinated through the Division of Human Genetics. The fellows participate in weekly conferences, Tumor Board and journal presentations, and have access to the research database. Patient sessions are four half days/week and the rotation for the resident includes learning experiences outside the clinic in pathology, radiology and cancer cytogenetics.
